NEW PATIENTS

We understand that your first visit to a doctor’s office can be a stressful situation, especially if you’re in pain, so we strive to make your experience as easy and stress-free as possible.

Please print and fill out the New Patient Forms prior to your appointment OR if you plan on filling the forms out in the office please arrive at least 15 minutes before your appointment time to ensure that you have a sufficient amount of time to complete them.


WHAT TO REMEMBER


ARRIVE ON TIME

Please arrive 15 minutes early to ensure that you have enough time to complete the required paperwork.


PICTURE I.D.

Driver’s license, passport, school ID, or any other form of picture identification accepted.


FORM OF PAYMENT

Payments for deductibles, co-pays, and uninsured services are due at the time services are rendered. We accept cash, checks, and credit cards.

LOOSE FITTING CLOTHES

Bring clothes that don’t restrict your movement. No dresses or skirts. Changing rooms are available.


PREVIOUS DIAGNOSTICS

Don’t forget to bring any previous diagnostic imaging including X-Rays or MRIs, related to your condition.


INSURANCE INFORMATION

If applicable, proper insurance documentation helps to expedite the registration process.


WHAT TO EXPECT


DAY 1

The initial visit is about 1 hour long and will consist of a thorough overview of your current condition as well as any other health concerns you may have. The doctor will perform a complete physical and neurological exam, specific orthopedic tests, posture evaluation, possibly make you perform some specific movements to assess how you’re body functions together, and X-rays if needed to get an accurate diagnosis of what is causing your symptoms. Dr. Long will then go over the exam findings with you, answer any questions or concerns you have, and explain your possible treatment options. If you are ready to begin treatment you will receive your first chiropractic adjustment that same day. *If chiropractic care is not suitable for your condition we will not hesitate to refer you to the appropriate health care provider so you can get the care you need.


DAY 2

The second session is thirty minutes long. We will go over how you responded to the first treatment, which gives us both a better idea of how long treatment should take. We will also go over your X-Rays, I’ll provide you any other questions or concerns you have regarding your treatment plan to ensure that we are both on the same page. (Each following visit after that will be 15 minutes long unless otherwise specified.)


INSURANCE INFORMATION


Beginning in 2021 our office became an OUT-OF-NETWORK PROVIDER with ALL insurance companies. This decision came about after several changes from the insurance companies that directly diminished the quality of care I was able to provide to my patients by dictating what it is I’m able to do for treatment, as well as who they deem worthy of receiving treatment under insurances’ strict standards of “medical necessity.”

For example, infants, pregnant women, and even children born premature with serious issues were only being allowed 4 visits per month, or they’d had to pay out of pocket to continue treatment since the insurance they pay for decided they weren’t in bad enough shape to cover their treatment. No matter what issue children would have, whether it be a baby getting dropped by another sibling or even rolling off the bed, it should have resolved completely within 1-3 months of care. Some things need long term care and regular maintenance to keep them from coming back, and unfortunately prevention and/or maintenance is not covered by insurance. You must be broken before it kicks into fix you.

By becoming an OUT-OF-NETWORK provider, I am able to continue to spend that extra time during our visits going over exercises and things you can do for yourself at home, and make sure you don’t have any questions or concerns.

I can and will still bill your insurance for you, and if you have a PPO plan it is likely you have OUT-OF-NETWORK BENEFITS that may still cover ALL or a portion of your visit, depending, of course, on if your deductible has been met and what your benefits are.

IF I were to stay as an IN-NETWORK provider, I would have to run a high volume practice where I would need to limit each visit to 5 minutes per patient, and would only have time to do a rushed adjustment and send you on your way with no extra therapy like electric muscle stimulation, corrective exercises, massage, etc. that I feel is important part of supporting the chiropractic adjustment, that way you have longer lasting results we’re all looking for.

Longevity Chiropractic has several affordable options that we’ve put together for both you and the whole family that allows you to continue to enjoy the high standard of chiropractic care we offer here at Longevity Chiropractic.

Insurance can AND ALWAYS WILL be confusing, so please reach out by phone/email if you have any questions regarding your specific situation or if you want to know what your Out-Of-Network benefits are at our office. We recommend calling on your end as well to verify your benefits, that way everyone’s on the same page and there are no surprises.
